Each player must have their release form turned-in to the the league prior to the first game.

NO TEAM will be able to participate without the roster and waiver complete.

• NON IYSA Teams Only (Boys NISL teams ONLY) - Need a copy of the US Club Insurance Form filled out by Club President or DOC
• IYSA Medical Release Form
